Forms

For adventures that we may take off campus that requires some form of transporation, We have to fill out a Motor Pool Request Form for campus vehicles. The pricing may have changed so check with Motor Pool via email or in the Public Safety folder in Outlook to get up-to-date pricing. N.B. you need to have taken the Defensive Driving course to use campus vehicles. You must email this form to public safety. In addition to this form you must also get another form available at the Public Safety office in Woods Penn.

For the treasurer and president, you will need Deposit/Withdrawal forms to request money from the organization account. The account can be accessed at the front desk of Lincoln Hall and the forms must be filled out before going to Lincoln Hall. This particular copy includes two separate forms so you can cut the sheet in half.
